Without completing this form, we will not be able to schedule and facilitate your accommodations and\/or modifications.<\/p>\n\n\n\n<p>This form will need to be completed for each course you are enrolled in, it can be submitted multiple times as needed. This form system is in a replacement for the previous emails notifying AR of quizzes or midterms. Following this submission, AR will email you your schedule for each class\u2019s exams.
